New or Renewal Applications
- Application: Applicant & Doctor fill out.
- Identification: 1 form of I.D. showing applicant's name & date of birth (Do not send an original document.)
Permit Application

Duplicate Applications (for lost or stolen permits)
- Application: Applicant fills out top portion. (Doctor signature is not required)
- Identification: 1 form of I.D. showing applicant's name & date of birth (Do not send an original document)
