Immunization Clinic


Purpose:
Protect children and their families from vaccine preventable diseases.
Hours:
8 AM to 4:00 PM Monday through Friday
Closed on Holidays
Cost:
Administration fee: $13.00 single shot; $15.00 multiple shots
$5.00 Duplication of immunization record
Basic Requirements:
  • Children under 19 years must be accompanied by a parent or legal guardian.
  • Bring your record of previous immunizations.
  • Vaccines are given to infants at approximately: 2, 4, 6, 12-18 months of age, after the 4th birthday and 7th Grade (11-15 years)
  • Flu and Pneumonia vaccines are offered every fall to at risk children & adults who qualify. Call Community Health Services Information & Referral Line at 441-8065 for more information. (NOTE: The Health Department uses vaccines provided by the state for the vaccination of children only. The Health Department does not provide routine immunizations to adults 19 years and older. Adult requiring vaccines should contact their physicians.)
