Chronic diseases, such as heart disease, cancer, and diabetes, are the leading causes of death and disability in the United States. In fact, 7 out of 10 deaths among Americans each year are from chronic diseases. Chronic disease makes learning and working more difficult, is expensive to treat, and puts a strain on our health care system. Although chronic diseases are among the most common and costly of all health problems, they are also among the most preventable.

Our Chronic Disease Prevention program promotes activities that encourage healthy living, embraces health screenings for early detection of disease, and encourages appropriate self-management of existing diseases. Program activities include community education on topics such as:

  • reducing the incidence of childhood obesity
  • increasing physical activity and developing healthy eating habits
  • diabetes prevention or control
  • learning to manage chronic health problems
  • colorectal cancer awareness

Program activities are successful because of the collaboration between the local hospitals, public health clinics, businesses, schools, the State Department of Health and Human Services, non-profit organizations, community and cultural centers, and other City departments who work together to improve the health of the community.
