Aged and Disabled Medicaid Waiver

Our mission is to enhance daily living, expand personal choices and educate the community to ensure the independence and full life of the people we serve. The Aged and Disabled Medicaid Waiver provides services and supports for individuals who meet nursing home criteria but would prefer to remain living at home or in an assisted living facility.

To be eligible for Medicaid Waiver services, the following criteria is required:

  • Must have physical and health needs that would otherwise require the individual to reside in a nursing home facility.
  • Must be Medicaid eligible, including clients who pay for a portion of their medical expenses on a “spend down”
  • A person must be 65 years or older
  • Can be safely served at home at a cost no more than Medicaid would pay for nursing facility care.
  • Agree to participate in needs assessments and develop an individualized service plan with the assigned Aging Specialist. 

Please note:  Medicaid helps people whose income and assets aren’t enough to pay for needed medical care. It is not the same as Medicare.

  • Respite Care Services
  • Personal Emergency Response System
  • Home Delivered Meals
  • Companion Services
