Community Health Improvement Planning

As a public health function, the Lincoln-Lancaster County Health Department serves as a chief health strategist to strengthen, support, and mobilize community partnerships to improve the health of the community. This is accomplished in part through the Community Health Assessment (CHA) and Community Health Improvement Planning (CHIP) process which cycles every three years.

The CHIP starts when partners utilize the community health data provided by LLCHD to set priorities and strategic focuses for engagement and improvement. CHIP work groups move planning to action over the course of the 3-year cycle. As a CHIP cycle is ending, a new Community Health Assessment is initiated. Both the CHA and the CHIP are done in collaboration with Bryan Health System and CHI Health St. Elizabeth and Nebraska Heart, and more than 75 partnering organizations throughout Lancaster County.

The Lincoln-Lancaster County Health Department convenes local experts, collaborators, and community partner organizations to conduct an environmental scan of the four strategic focus areas.  The community team members and partnering organizations set priorities for the next 1-3 years, using the results of the environmental scan. Every quarter, each objective and new action strategy is reviewed and evaluated for updates.
