Chronic disease continues to increase in prevalence in both our community and nationwide. About 38 million Americans have diabetes and 1 in 5  don't know it, and heart disease is the leading cause of death, responsible for 1 in every 5 deaths (Centers for Disease Control). This CHIP priority will focus on aligning worksite wellness program efforts to effectively reach larger populations for promoting healthy lifestyle actions and risk reduction for diabetes and heart disease.

Action

  1. Madonna Fit for Work and Nebraska Safety Council has partnered to share de-identified data (such as blood pressure, blood sugar, body fat around the waist, and cholesterol or triglyceride levels) for assessing community health and the development of trend indicators.

  2. Currently collecting data to identify best practices that may reduce diabetes, heart disease, and unhealthy behaviors through worksite wellness opportunities that can be piloted and measured.

  3. Make a collective impact in Lancaster County on chronic disease prevention efforts through worksite wellness initiatives, including education, community resources (such as MYLINK), screening, and referrals.
