Community Health Assessment

As one of our public health functions the Lincoln-Lancaster County Health Department routinely assesses the health status in the community. We do so to analyze trends, inform and educate ourselves and our partners, target services and evaluate the progress of our many programs.

In addition to this ongoing assessment, every three years we engage a far more in-depth Community Health Assessment (CHA), in collaboration with area hospitals and the broader public health infrastructure. The last Community Health Assessment was in 2015. Currently, the Health Department is once again engaging with partners and the community in a robust health status assessment for Lincoln and Lancaster County. Bryan Health Systems and CHI St. Elizabeth are key partners in a new approach that will allow even more focused efforts to address health outcomes of concern and to achieve health equity.

Community Health Assessment  2024 Modules

The 2024 Community Health Assessment is composed of many parts, including the modules below. By viewing the videos in the module and completing the surveys related to those videos, you'll be providing valuable community input contributing to generating community health priorities. The modules will be closed Friday, August 2nd, 2024 at midnight.
